NC HFWTP Process
Our implementation specialists credential coaches and their team members in the HFW model.
Once coaches are credentialed, implementation specialists shift to a monitoring role.
Credentialing
Implementation specialists credential teams in the HFW model.
All team members undergo Foundational Training.
The implementation specialist credentials the coach.
The coach credentials the other HFW team members, with the implementation specialist providing oversight based on where the coach is in their own credentialing process.
Each team member has unique skills to master for their roles, and thus unique activities to perform during the process.
Re-credentialing is conducted every two years after the initial credentialing.
Monitoring
Implementation specialists continue working with coaches after credentialing is complete, on activities designed to assure fidelity to and growth of the HFW model.
Coaches provide regular group coaching to their HFW team members, to support their teams in reaching competency and to prevent drift from HFW model.
Implementation specialists periodically observe these sessions, to assess and provide recommendations on coaching skills and team building.
Implementation specialists periodically audit a coach’s records, which should include:
- the Crisis Plan,
- Strengths, Needs Cultural Discovery (SNCD)
- the Wrap Plan
- notes from child and family team meetings
Team facilitators collect data from the following sources:
- Wraparound Fidelity Index (WFI-EZ) forms
- Chart forms
- Initial documentation form
- Engagement form
- Team meeting update forms
- Transition cover sheet
- Post-engagement form
With assistance from implementation specialists, coaches use this data, in combination with other site records and their own observations, to improve:
- outcomes
- team member skills
- overall team functioning
